Summer Camp Forms
Below are links to the forms that we use for summer camp. The waiver and the health form are required by all children. This year, we have included a link to the CT Assessment form, which is the standard health form for schools in Connecticut and one most parents already have at home. Do remember to fill out all of the pages. A separate page for immunizations is accepted.
If your child does not attend school in Connecticut, please download and complete the WNC health form. Remember to include current information about allergies and immunizations.
The Authorization of Medication at Camp packet is for any child who may require medicine will at camp such as an epi pen.
